Affinity Home Health Care has provided quality home care services in Pasadena and the surrounding Gulf Coast area for over 13 years. Family owned and operated, we truly take pride in our agency and consider our clients “family.” So, what sets us apart from other home care agencies and why should you choose Affinity for your healthcare needs?

Affinity Home Health Care is staffed with nurses and other medical professionals who are specifically trained to provide care in your home.

Affinity Home Health Care services are provided for individuals with health conditions which make it difficult, if not impossible, for them to travel outside the home to receive care or treatment. We serve those who need care for chronic diseases, are temporarily disabled, and those who require long term care.

In addition to our skilled nursing and therapy services, we offer specialized services such as:

    *Chronic Disease Management – CHF, COPD, Diabetes and Wound Care Pathways to monitor and treat our clients;
    *Value-added services – medication “cross reference” checks and reconciliation in partnership with the PCP and other physicians involved in the care of the client and tele-monitoring services to help prevent re-hospitalizations;
    *Specialized orthopedic services to evaluate client's needs prior to surgery and rehabilitative nursing care post surgery
    *Bi-lingual nurses (Spanish)

How Much Does Medicare Prescription Drug Coverage Cost?

Each plan can vary in cost and drugs covered. The Medicare Plan Finder can help you find and compare plans in your area. Your Part D monthly premium could be higher based on your income. This includes Part D coverage you get from a Medicare Prescription Drug Plan, or a Medicare Advantage Plan or Medicare Cost Plan that includes Medicare prescription drug coverage. If your modified adjusted gross income as reported on your IRS tax return from 2 years ago (the most recent tax return information provided to Social Security by the IRS) is above a certain amount, you will pay a higher monthly premium.
